Student Accident Insurance Plan

Dear Parent or Guardian:

The health and safety of your child is important to us.  Therefore, we participate in a Catastrophic Student Accident Insurance Program.  The Program pays uninsured covered medical expenses that result from school sponsored and supervised activities.  The Program pays once expenses exceed $50,000 and then up to $5,000,000.  The policy is underwritten by Monumental Life Insurance Company.  The cost of this coverage is paid by the school.

In addition, we secured the availability of optional (parent/guardian paid) Student Accident Insurance for your child for the School Year.  The insurer is Sentry Life Insurance Company.  The coverage pays for uninsured “covered medical expenses” for one year provided (1) the initial treatment is rendered within 60 days of the date of injury and (2) the accident results from a “covered activity”.

The optional coverage, like the Catastrophic Program, is excess of other collectible insurance and therefore only pays when there is no other available insurance or for amounts not paid by another health insurance policy.  If there is other coverage, you still need to comply with the terms of the other insurance in order for the optional and Catastrophic coverage to apply.

The “covered medical expenses” vary by plan type.  There are three plan options: (1) Gold; (2) Silver; and (3) Bronze.  Gold offers the highest limit and broadest terms.  In addition, an optional Dental Benefit is available.  The Gold option is recommended, even though the amount of coverage might overlap with the Catastrophic Program.

There are four types of “covered activities”: (1) School-Time Only; (2) 24-Hour; (3) Football Only; and (4) Spring/Summer Weight Conditioning Training Only.  The Football and Weight Conditioning options can be purchased separately or in any combination with the School-Time Only or 24-Hour coverage.

The School-Time Only option provides coverage for injuries incurred during the hours and days when school is in session and while attending or participating in school sponsored and supervised activities on or off school premises, except for overnight field trips.  The annual cost of the School-Time Only option for Gold coverage is $31.00.

The 24-Hour option provides coverage during the weekends and vacation periods including the entire summer. Students are protected while at Home or away, any place, anytime, anywhere.  The annual cost of the 24-Hour option for Gold coverage is $125.00.

The School-Time Only and 24-Hour coverage includes all interscholastic sports except football and spring and summer weight conditioning, unless separately elected.  The annual cost of the Football option for Gold coverage is $163.00.  The Weight Conditioning charge is $57.00.

In order to enroll, please go to the website www.k12specialmarkets.com. Select Archdiocese of Baltimore from the drop-down menu.  Then from that list select our school.
