Nurse

ICS employs a registered nurse, on duty five days per week in the health room. Students must ask permission of their teacher and receive a pass before going to the Health Room. The nurse will attend to the student and decide if the child is to return to class or be sent home. NO student is ever sent home alone. An adult must come to take the student home. If any injury to the head or any other serious injury occurs to a student, the nurse will call the parent/guardian and send home a report. If the parent/guardian or alternative contact person cannot be reached in an emergency, the school will contact police/ambulance for assistance.

Parents are required to complete a health form listing any allergies, serious medical conditions, medications, and emergency contact information. The school must be notified of changes that occur during the school year.

ALL MEDICAL FORMS MUST BE COMPLETED AND SIGNED BY BOTH YOU AND YOUR PHYSICAN BY AUGUST 15 EACH SCHOOL YEAR


All NEW STUDENTS K-8 must turn in a New Student Health Registration , I-896 Immunization Record, and Dental Form.Health Inventory , I-896 Immunization Record

All PRESCHOOL students must turn in a New Student Health Registration ,   I-896 Immunization Record, Dental Form, Lead Form, and an All About Me Form.Preschool Health Inventory,

Any Student requiring medication must have the Medication Order Form turned in by the first day of school. This includes daily medications and those used on an as needed basis. The Over the Counter Medication Form is optional. If you do choose to use it, it must be signed by BOTH YOU AND YOUR CHILD’S PHYSICIAN.
