Ice Cream Coupon Book Order Form
REMINDER FROM THE NURSES:
Any child with a medical condition, allergy, or food allergy, that may require the use of medicine (prescription and/or over the counter) during school hours must get the physician's orders signed, and medicine to the nurse. These forms, entitled "Medication Order Form" and "Emergency Health Care Plan" may be found on the school website by going to the heading Faculty & Staff and clicking on Nurse. It is imperative that those students requiring daily medications, inhalers, Benadryl, or an Epi-pen for allergies and/or asthma have these two forms filled out as soon as possible. No medication, prescription or over the counter can be given without a physician’s order.
The policy for over the counter (OTC) medication administration in the school clinic is written according to the standards that are mandated by the Nurse Practice Act of Maryland. ALL Over the Counter medications, including cough drops, can only be dispensed with the parent’s written permission AND the signature of the student’s physician on the “Over the Counter Meds” form.
Any medication not on the OTC form, such as eye drops, that is needed during school hours, requires the completion of the Medication Order Form.
A one time dose can be administered with parental permission, but no subsequent medication will be dispensed without the physician signature.
Please send in ALL medications and Orders as soon as possible.
